What Is on a Real Pharmacy Receipt?

US pharmacy receipts follow a regulated structure so insurers, FSA/HSA administrators, and pharmacists can all read the same document the same way.

Rx number and NDC code

Each prescription is assigned a store-specific Rx number (often 6-7 digits), and the drug itself carries an NDC (National Drug Code) printed in a 5-4-2 or 5-3-2 digit format that identifies the manufacturer, product, and package size.

Days supply as its own line

Pharmacy receipts print the days supply (for example, 30 or 90) separately from quantity, since insurers use days supply - not pill count - to check refill timing and prevent early-refill rejections.

Copay vs. usual and customary price

Insured receipts show the copay actually charged next to the usual and customary (cash) price, so the patient can see what insurance saved. Cash-pay receipts show only the U&C price with no insurance adjustment line.

Pharmacist verification line

Most pharmacy systems print the verifying pharmacist's initials or name on the receipt or attached information sheet, confirming the fill was checked before dispensing - a legal requirement in most US states.

Insurance and copay identifiers

Insured claims show the insurance plan name, BIN/PCN/group number, and member ID used to process the claim, alongside the patient responsibility amount.

Rx and front-store items often separate

Because prescriptions are billed through a separate insurance adjudication system, many pharmacies print the Rx purchase on its own receipt, distinct from OTC and front-of-store items paid together at the same register.
