What Is on a Real Itemized Receipt?

FSA administrators, insurers, and expense teams reject receipts that are missing specific fields. These are the elements that make a receipt itemized - and acceptable as substantiation.

Merchant name and contact details

The header identifies the vendor or provider by name with address or phone details. Substantiation rules require documentation from an independent third party, so the merchant identity is the first thing reviewers check.

Date of sale or service

The transaction date must appear on the receipt itself. FSA and HSA claims are reimbursed against the date the expense was incurred, not the date it was paid or submitted, so a receipt without a date fails review.

A description on every line

Each product or service gets its own line with a readable description - this is the defining feature of an itemized receipt. A card slip or summary receipt showing only a total does not count as itemized.

Quantity and unit price per line

Detailed receipts show the quantity and unit price extended to a line amount, which lets insurance adjusters value individual items in a loss claim and lets expense reviewers separate reimbursable items from personal ones.

Tax broken out from the subtotal

Sales tax prints as its own line after the subtotal, sometimes split by rate (state plus local). Some employer expense policies and tax filings need the tax amount separated from the merchandise total.

Amount paid and payment method

The total, the payment method, and the amount actually paid close out the receipt. IRS substantiation guidance requires the expense amount, and reviewers use the payment line to distinguish a paid receipt from an unpaid bill or estimate.
