What Is on a Real Rx Prescription Receipt?
An Rx receipt is more clinical than a retail receipt - it exists to document exactly what drug, in what form, was dispensed and by whom, for insurance, pharmacy law, and patient safety reasons.
NDC in 5-4-2 or 5-3-2 format
The National Drug Code identifies labeler, product, and package size in segments (commonly printed as 5-4-2 digits, e.g., 00000-0000-00). Insurance claims and FSA/HSA reviewers use this code, not just the drug name, to verify what was dispensed.
Days supply, separate from quantity
Days supply (e.g., 30 or 90) is printed distinctly from the pill or unit count, since PBMs use days supply to calculate refill-too-soon rejections and insurance billing cycles.
DAW code on the fill record
Dispense As Written codes (0-9) indicate whether a generic substitution was allowed or blocked - DAW 0 means substitution permitted, DAW 1 means the prescriber required the brand product. This code affects the price and appears on the detailed fill record.
Two separate NPI numbers
A complete Rx record shows the prescriber NPI (the doctor who wrote the script) and, separately, the pharmacist or pharmacy NPI (who filled and verified it) - these are distinct identifiers required for insurance claims processing.
Refill number and refills remaining
The receipt shows which fill number this is (original or refill #) and how many refills remain as authorized by the prescriber, since pharmacies cannot dispense beyond the authorized refill count without a new script.
Copay vs. total drug cost
Insured Rx receipts show the copay charged separately from the full cost of the drug, so patients and FSA/HSA administrators can see what insurance covered versus what was paid out of pocket.
